What forms do I need to fill out for my first dental visit?
New patients at Willow Family Dentistry need to complete four forms before their first visit: New Patient Registration Form (personal and contact information), Medical History Form (health conditions, medications, allergies), HIPAA Privacy Notice (how we protect your information), and Financial Policy (payment and insurance details). You can download these forms online, complete them at our office, or request them by email.
